Consequently, many of its toxicities are cumulative and depend on the total dose taken.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Pulmonary Toxicity<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Amiodarone-induced pulmonary toxicity is the most severe and potentially fatal complication, affecting approximately 1% to 5% of patients.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Pathology:<\/strong> It most commonly presents as interstitial pneumonitis (inflammation of the lung tissue) or organizing pneumonia, which can progress to irreversible pulmonary fibrosis (scarring) if undetected.<\/li>\n\n\n\n<li class=\"\"><strong>Symptoms:<\/strong> Patients typically experience an insidious onset of a dry, non-productive cough, shortness of breath, and occasionally a low-grade fever.<\/li>\n\n\n\n<li class=\"\"><strong>Monitoring:<\/strong> Baseline chest X-rays and pulmonary function tests (specifically checking gas diffusion capacity) are required before starting the drug, with annual imaging thereafter.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Thyroid Dysfunction<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Because amiodarone structurally resembles thyroxine and consists of about 37% iodine by weight, it profoundly impacts thyroid hormone metabolism.<sup><\/sup><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Hypothyroidism (underactive thyroid):<\/strong> This is the most common adverse effect, occurring in up to 20% of patients. It causes fatigue, weight gain, and cold intolerance. It can usually be managed by adding a thyroid hormone replacement (like levothyroxine) without needing to stop the amiodarone.<\/li>\n\n\n\n<li class=\"\"><strong>Hyperthyroidism (overactive thyroid):<\/strong> Occurring in 1% to 2% of patients, this is more difficult to manage and can dangerously exacerbate underlying heart conditions by causing rapid heart rates and weight loss.
