To understand how statins work, it helps to look at how a dangerous plaque is built. A &#8220;vulnerable&#8221; plaque has a large, soft lipid (fat) core covered by a thin, inflamed outer shell called a fibrous cap. If that delicate cap tears, the fatty contents spill into the bloodstream, immediately triggering a blood clot that can cause a heart attack or stroke.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Stabilization: The Real Superpower<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">When you take a statin, particularly at a high-intensity dose, it aggressively lowers the low-density lipoprotein (LDL) cholesterol circulating in your blood. Deprived of new cholesterol, the body begins to pull lipids <em>out<\/em> of the plaque&#8217;s soft core.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This process changes the physical architecture of the plaque:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>The core solidifies:<\/strong> The dangerous &#8220;necrotic core&#8221; becomes smaller, denser, and often calcified.<\/li>\n\n\n\n<li class=\"\"><strong>The cap thickens:<\/strong> The fibrous cap covering the plaque becomes significantly thicker and tougher.<\/li>\n\n\n\n<li class=\"\"><strong>Inflammation drops:<\/strong> Statins have &#8220;pleiotropic&#8221; (non-cholesterol-lowering) effects that actively calm inflammation within the artery wall.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The plaque is still there, but it has been converted from a fragile, volatile blister into a hardened, stable scar. A stable plaque is far less likely to rupture.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Regression: Modest Shrinkage<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">While stabilization happens relatively quickly and prevents heart attacks, actual physical shrinkage of the plaque takes longer and is anatomically modest.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Clinical trials using intravascular ultrasound (IVUS) \u2014 where a tiny probe is threaded directly into the coronary arteries \u2014 have proven that driving LDL cholesterol to very low levels can actually reverse plaque volume. However, the reduction is usually a matter of a few percentage points.
