There is right to left shunting across the ventricular septal defect causing cyanosis. In infancy, Tetralogy of Fallot can cause cyanotic spells. Cyanotic spells can be prevented to a certain extend by giving propranolol. A palliative modified Blalock-Taussig shunt is also useful to prevent spells.
