Idiopathic VT

Those with ventricular tachycardia and having no diagnostic abnormalities on magnetic resonance imaging are included in idiopathic ventricular tachycardia.

Sites of origin of idiopathic VT:

  1. Fascicular VT
  2. Outflow tract VT
  3. Perivenous VT – in the branches of the coronary venous system
  4. Aortic cusp VT

Ablation from the aortic cusp need not mean that it is originating from the aortic cusp as the depth of lesion may extend beyond the aortic cusp.

While ablating aortic cusp VT, it is mandatory to do a coronary angiogram and delineate coronary anatomy and the aortic cusp before ablating.
