Modern heart failure management, particularly the integration of the “Four Pillars” of Guideline-Directed Medical Therapy (GDMT)—ARNI, SGLT2 inhibitors, Beta-blockers, and MRAs—has fundamentally changed the landscape of ICD eligibility.
