Tall peaked P waves, also known as P pulmonale, indicate right atrial enlargement. On standard ECG paper, height should be more than 2.5 mm.

Left atrial enlargement is seen as wide, notched P waves, with width of more than 2.5 mm, known as P mitrale. In biatrial enlargement, P wave width is more than 2.5 mm and height more than 2.5 mm.
