They obtained baseline echocardiographic measurements including left ventricular ejection fraction and global longitudinal strain in 450 acute leukemia patients treated with anthracyclines. A 21 point risk score was generated by evaluating potential risk factors for heart failure using Fine and Gray&#8217;s regression analysis.<\/p>\n<p>Six variables were selected based on their statistical significance and clinical relevance (assigned score points for each in brackets):<\/p>\n<ol>\n<li>Age above 60 years (<span style=\"color: #ff0000;\">Score:<\/span> <strong><span style=\"color: #ff0000;\">1<\/span><\/strong>)<\/li>\n<li>Cumulative anthracycline dose of 250 mg per sq. m or more (<span style=\"color: #ff0000;\">Score:&nbsp;<\/span><strong><span style=\"color: #ff0000;\">2<\/span><\/strong>)<\/li>\n<li>Presence of acute myeloid leukemia (<span style=\"color: #ff0000;\">Score: <\/span><strong><span style=\"color: #ff0000;\">4<\/span><\/strong>)<\/li>\n<li>Pre-existing cardiovascular disease (<span style=\"color: #ff0000;\">Score:&nbsp;<\/span><strong><span style=\"color: #ff0000;\">4<\/span><\/strong>)<\/li>\n<li>Baseline left ventricular ejection fraction below 50% (<span style=\"color: #ff0000;\">Score:&nbsp;<\/span><strong><span style=\"color: #ff0000;\">4<\/span><\/strong>)<\/li>\n<li>Global longitudinal strain more than -15% (<span style=\"color: #ff0000;\">Score: <\/span><strong><span style=\"color: #ff0000;\">6<\/span><\/strong>)<\/li>\n<\/ol>\n<p>Sum of scores for each patient was calculated and the cumulative score would range from 0 to 21. They were further subclassified into three risk groups (one year cumulative heart failure risk in brackets):<\/p>\n<ol>\n<li>Low risk: Score 0-6 (<span style=\"color: #0000ff;\">Cumulative heart failure risk:<\/span> <strong><span style=\"color: #0000ff;\">1%<\/span><\/strong>)<\/li>\n<li>Moderate risk: Score 7-13 (<span style=\"color: #0000ff;\">Cumulative heart failure risk:&nbsp;<\/span><strong><span style=\"color: #0000ff;\">13.6%<\/span><\/strong>)<\/li>\n<li>High risk: Score 14-21 (<span style=\"color: #0000ff;\">Cumulative heart failure risk:&nbsp;<\/span><strong><span style=\"color: #0000ff;\">35%<\/span><\/strong>)<\/li>\n<\/ol>\n<p>The heart failure risk score was also predictive of all cause mortality.
