{"id":30219,"date":"2020-01-05T07:51:03","date_gmt":"2020-01-05T07:51:03","guid":{"rendered":"https:\/\/johnsonfrancis.org\/professional\/?p=30219"},"modified":"2020-01-05T07:51:03","modified_gmt":"2020-01-05T07:51:03","slug":"rox-index","status":"publish","type":"post","link":"https:\/\/johnsonfrancis.org\/professional\/rox-index\/","title":{"rendered":"ROX index"},"content":{"rendered":"<h2><span style=\"color: #008000;\">ROX index<\/span><\/h2>\n<p><iframe loading=\"lazy\" width=\"560\" height=\"315\" src=\"https:\/\/www.youtube.com\/embed\/EB8L1WECN0A\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen=\"\"><\/iframe><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-41146\" src=\"https:\/\/johnsonfrancis.org\/professional\/wp-content\/uploads\/2020\/01\/ROX-Index1.jpg\" alt=\"\" width=\"763\" height=\"346\"><\/p>\n<p>ROX index is used to predict the success (and failure) of high-flow nasal cannula (HFNC). <span style=\"color: #ff00ff;\">ROX index is the ratio of pulse oximetry\/fraction of inspired oxygen (FIO<sub>2<\/sub>) to respiratory rate. <span style=\"color: #0000ff;\">The acronym ROX stands for <span style=\"color: #ff0000;\">R<\/span>espiratory rate and <span style=\"color: #ff0000;\">OX<\/span>ygenation.<\/span><\/span><\/p>\n<figure id=\"attachment_41136\" aria-describedby=\"caption-attachment-41136\" style=\"width: 538px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-41136\" src=\"https:\/\/johnsonfrancis.org\/professional\/wp-content\/uploads\/2020\/01\/ROX-Index-1.jpg\" alt=\"ROX Index\" width=\"538\" height=\"595\"><figcaption id=\"caption-attachment-41136\" class=\"wp-caption-text\">ROX Index<\/figcaption><\/figure>\n<p>ROX index of 4.88 or more measured after 12 hours of HFNC was associated with a lower risk of mechanical ventilation. This was noted in a study conducted on 157 patients with pneumonia [1]. It was a 4 year observation two center cohort study including patients with severe pneumonia treated with HFNC. 44 patients who needed mechanical ventilation were considered as HFNC failures.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-41139\" src=\"https:\/\/johnsonfrancis.org\/professional\/wp-content\/uploads\/2020\/01\/ROX-Index2.jpg\" alt=\"\" width=\"724\" height=\"382\"><\/p>\n<p>Another study involving 191 patients with pneumonia treated with HFNC showed that prediction accuracy of ROX index increased over time [2]. An index 4.88 or above at two hours had a hazard ratio of 0,434, at 6 hours had hazard ratio of 0,304 and at 12 hours of 0.291. That is a lower risk of intubation while on HFNC. Predictors of HFNC failure were an index less than 2.85 at 2 hours, less than 3.47 at 6 hours and less than 3.85 at 12 hours after initiation of HFNC.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-41143\" src=\"https:\/\/johnsonfrancis.org\/professional\/wp-content\/uploads\/2020\/01\/ROX-Index3.jpg\" alt=\"\" width=\"344\" height=\"392\"><\/p>\n<p>HFNC can delivered up to 60 liters of flow with humidified and warmed oxygen using a special bedside equipment and a large nasal cannula. Fraction of inspired oxygen concentration can be increased up 100% from the 21% available in room air. Due to the large flow, dilution by room air as occurs with conventional nasal oxygen cannula is minimized.
