{"id":6029,"date":"2014-12-23T02:33:53","date_gmt":"2014-12-23T02:33:53","guid":{"rendered":"http:\/\/cardiophile.org\/?p=6029"},"modified":"2014-12-23T02:33:53","modified_gmt":"2014-12-23T02:33:53","slug":"what-is-aldosterone-escape","status":"publish","type":"post","link":"https:\/\/johnsonfrancis.org\/professional\/what-is-aldosterone-escape\/","title":{"rendered":"What is aldosterone escape?"},"content":{"rendered":"<h2><span style=\"color: #008000;\">What is aldosterone escape?<\/span><\/h2>\n<p><iframe loading=\"lazy\" width=\"560\" height=\"315\" src=\"https:\/\/www.youtube.com\/embed\/Z1_E6Lifd04\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen=\"\"><\/iframe><\/p>\n<p><strong><span style=\"color: #0000ff;\">Abstract:<\/span> <\/strong><\/p>\n<ol>\n<li><span style=\"color: #008000;\"><em>The term aldosterone escape in primary hyperaldosteronism is escape from the salt and water retaining effects of aldosterone. <\/em><\/span><\/li>\n<li><span style=\"color: #008000;\"><em>In the treatment of heart failure with angiotensin converting enzyme inhibitors, it is the breakthrough occurrence of aldosterone effect.<\/em><\/span><\/li>\n<\/ol>\n<p><span style=\"color: #0000ff;\"><strong>Different types<\/strong><\/span><\/p>\n<p>The term has been used in two different settings:<\/p>\n<ol>\n<li>Aldosterone escape in primary hyperaldosteronism<\/li>\n<li>Aldosterone escape in the treatment of heart failure with angiotensin converting enzyme inhibitors<\/li>\n<\/ol>\n<p><span style=\"color: #0000ff;\"><strong>In primary hyperaldosteronism<\/strong><\/span><\/p>\n<p>In primary hyperaldosteronism, it is the escape from the salt and water retaining effect of excessive levels of aldosterone.<sup>1<\/sup> This phenomenon is considered to be an important homeostatic mechanism and thought to be dependent on nitric oxide.<sup>2&nbsp;<\/sup>Natriuresis produced by elevated levels of atrial natriuretic peptides is another&nbsp;proposed mechanism for escape from the salt retention in this situation.<sup>3<\/sup> Pressure natriuresis is another proposed mechanism for aldosterone escape, with increased renal arterial pressure induced natriuresis.<sup>4<\/sup>&nbsp;Aldosterone escape in the setting of primary hyperaldosteronism may have an important role in reducing the severity of hypertension in the condition. <span style=\"color: #0000ff;\">Aldosterone escape is the&nbsp;reason for edema not being a prominent manifestation of primary hyperaldosteronism.<\/span><\/p>\n<p>This mechanism does not work in the aldosterone excess of edematous conditions like congestive heart failure and cirrhosis liver, possibly due to the renal hypoperfusion, which is not a feature of primary aldosteronism.<\/p>\n<p><span style=\"color: #0000ff;\"><strong>In the treatment of heart failure with angiotensin converting enzyme inhibitors<\/strong><\/span><\/p>\n<p>In chronic treatment of heart failure with angiotensin converting enzyme inhibitors is thought to be mediated by potassium dependent aldosterone secretion, which is unrelated to the renin-angiotensin-aldosterone system (RAAS). Potassium is one of the important stimuli for secretion of aldosterone from the zona glomerulosa of the adrenal cortex.<sup>5<\/sup> This type in which plasma aldosterone levels return to pretreatment levels in over a third of patients treated with angiotensin converting enzyme inhibitors or angiotensin receptor blockers is better termed<span style=\"color: #ff0000;\"> <em>&#8216;aldosterone breakthrough&#8217;<\/em> <\/span>according to some authors.<sup>6<\/sup> This is one reason for the current practice of adding a mineralocorticoid antagonist to angiotensin converting enzyme inhibitor or angiotensin receptor blocker in the treatment of heart failure.<\/p>\n<p><span style=\"color: #0000ff;\"><strong>References<\/strong><\/span><\/p>\n<ol>\n<li>Knox FG, Burnett JC Jr, Kohan DE, Spielman WS, Strand JC.
