If you answer <strong>YES<\/strong> to any step, the diagnosis is <strong>VT<\/strong>.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Step<\/strong><\/td><td><strong>Criteria<\/strong><\/td><td><strong>Significance<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>1<\/strong><\/td><td><strong>Absence of RS complex<\/strong> in all precordial leads (V1-V6)?<\/td><td>This is &#8220;concordance.&#8221; If all leads are entirely positive (R) or entirely negative (QS), it\u2019s VT.<\/td><\/tr><tr><td><strong>2<\/strong><\/td><td><strong>RS interval &gt; 100ms<\/strong> in any precordial lead?<\/td><td>Measure from the start of the R wave to the deepest part (nadir) of the S wave. If &gt;100ms, it&#8217;s VT.<\/td><\/tr><tr><td><strong>3<\/strong><\/td><td><strong>Atrioventricular (AV) Dissociation?<\/strong><\/td><td>Look for P waves &#8220;marching&#8221; independently of the QRS. This is a &#8220;smoking gun&#8221; for VT.<\/td><\/tr><tr><td><strong>4<\/strong><\/td><td><strong>Morphology Criteria<\/strong> for VT?<\/td><td>Specific &#8220;classic&#8221; VT shapes (e.g., &#8220;Left Rabbit Ear&#8221; taller than the right in V1).<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Reference<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li class=\"\">Brugada P, Brugada J, Mont L, Smeets J, Andries EW. <a href=\"https:\/\/www.ahajournals.org\/doi\/pdf\/10.1161\/01.cir.83.5.1649\">A new approach to the differential diagnosis of a regular tachycardia with a wide QRS complex<\/a>.
