While the field is vast, three lesions every clinician\u2014from neonatologists to family practitioners\u2014should be able to recognize on an echo report or clinical presentation.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">1. Ventricular Septal Defect (VSD)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>Ventricular Septal Defect<\/strong> is the most common congenital heart lesion, accounting for nearly 30% of all CHD. It consists of a hole in the septum separating the left and right ventricles.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Echo Hallmark:<\/strong> A high-velocity jet of blood crossing from the left ventricle (high pressure) to the right ventricle (low pressure) during systole.<\/li>\n\n\n\n<li class=\"\"><strong>Clinician Note:<\/strong> Look for <strong>left-heart enlargement<\/strong> (LA and LV dilation) on the report. This indicates &#8220;volume overload&#8221; because the extra blood sent to the lungs eventually returns to the left side of the heart.<\/li>\n\n\n\n<li class=\"\"><strong>Clinical Pearl:<\/strong> Small VSDs often produce a <em>louder<\/em> murmur (harsh, holosystolic) than large ones because the high pressure forced through a tiny hole creates more turbulence.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">2. Tetralogy of Fallot (TOF)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Tetralogy of Fallot<\/strong> is the most common cyanotic (&#8220;blue baby&#8221;) heart defect. It is a &#8220;conotruncal&#8221; anomaly, meaning it stems from a single developmental error in the heart&#8217;s outflow tract.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is defined by four classic features (often remembered by the acronym <strong>PROVe<\/strong>):<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li class=\"\"><strong>P<\/strong>ulmonary Stenosis (Right Ventricular Outflow Tract Obstruction)<\/li>\n\n\n\n<li class=\"\"><strong>R<\/strong>ight Ventricular Hypertrophy<\/li>\n\n\n\n<li class=\"\"><strong>O<\/strong>verriding Aorta (The aorta sits over the VSD instead of the left ventricle)<\/li>\n\n\n\n<li class=\"\"><strong>V<\/strong>entricular Septal Defect<\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Echo Hallmark:<\/strong> A large &#8220;malalignment&#8221; VSD with the aortic root &#8220;straddling&#8221; or overriding the septal crest.<\/li>\n\n\n\n<li class=\"\"><strong>Clinician Note:<\/strong> The severity of the child&#8217;s cyanosis is dictated by the degree of <strong>Pulmonary Stenosis<\/strong>. The tighter the exit to the lungs, the more deoxygenated blood is forced through the VSD into the aorta.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">3. Transposition of the Great Arteries (d-TGA)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>d-Transposition of the Great Arteries<\/strong> is a true cardiac emergency and the most common cyanotic lesion presenting in the <strong>first week of life<\/strong>. In TGA, the &#8220;plumbing&#8221; is switched: the aorta arises from the right ventricle, and the pulmonary artery arises from the left ventricle.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>The Result:<\/strong> Two separate, parallel circuits. Oxygenated blood just goes back to the lungs; deoxygenated blood just keeps circulating through the body. The baby survives only if there is a &#8220;mixing&#8221; site like a <strong>Patent Ductus Arteriosus (PDA)<\/strong> or <strong>Atrial Septal Defect (ASD)<\/strong>.<\/li>\n\n\n\n<li class=\"\"><strong>Echo Hallmark:<\/strong> The &#8220;Egg on a Side&#8221; appearance on X-ray, but on echo, the classic sign is seeing the two great vessels running <strong>parallel<\/strong> to each other rather than crossing in their normal &#8220;X&#8221; configuration. <strong>The image shows PA from LV, recognized by its bifurcation<\/strong>.<\/li>\n\n\n\n<li class=\"\"><strong>Clinician Note:<\/strong> If you suspect TGA in a neonate, the immediate priority is keeping the ductus arteriosus open with <strong>Prostaglandin (PGE1)<\/strong> until surgery can be performed.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Summary for the Clinician<\/h3>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Lesion<\/strong><\/td><td><strong>Cyanotic?<\/strong><\/td><td><strong>Key Echo Finding<\/strong><\/td><td><strong>Clinical Priority<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>VSD<\/strong><\/td><td>No<\/td><td>Left-to-right systolic jet<\/td><td>Monitor for heart failure\/weight gain<\/td><\/tr><tr><td><strong>TOF<\/strong><\/td><td>Yes<\/td><td>Overriding aorta + RVOT obstruction<\/td><td>Manage &#8220;Tet Spells&#8221; (knees-to-chest)<\/td><\/tr><tr><td><strong>d-TGA<\/strong><\/td><td>Yes (Severe)<\/td><td>Parallel great vessels<\/td><td>Maintain ductal patency (Prostaglandins)<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Pediatric echocardiography is the gold standard for diagnosing Congenital Heart Disease (CHD).
