Point-of-Care Ultrasound (POCUS) and echocardiography provide a non-invasive &#8220;window&#8221; into the patient&#8217;s physiology, allowing you to categorize shock into four main types: <strong>Hypovolemic, Cardiogenic, Obstructive,<\/strong> and <strong>Distributive.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most common framework for this is the <strong>RUSH Protocol<\/strong> (Rapid Ultrasound for Shock and Hypotension), which systematically evaluates the <strong>Pump<\/strong>, the <strong>Tank<\/strong>, and the <strong>Pipes<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">1. The Pump: Cardiac Assessment<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Using the Parasternal Long Axis (PLAX), Parasternal Short Axis (PSAX), and Apical 4-Chamber (A4C) views, you can assess the heart&#8217;s function and structure.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Shock Type<\/strong><\/td><td><strong>POCUS Findings (Heart)<\/strong><\/td><td><strong>Specific Signs<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>Hypovolemic<\/strong><\/td><td>Hyperdynamic (vigorous contraction)<\/td><td>&#8220;Kissing papillary muscles&#8221; (LV walls touch in systole)<\/td><\/tr><tr><td><strong>Cardiogenic<\/strong><\/td><td>Hypodynamic (poor LV\/RV contractility)<\/td><td>Dilated LV, poor mitral valve excursion (EPSS &gt; 7mm)<\/td><\/tr><tr><td><strong>Obstructive<\/strong><\/td><td>RV Strain \/ Tamponade signs<\/td><td><strong>D-sign<\/strong> (septal flattening), RV &gt; LV size, Pericardial effusion<\/td><\/tr><tr><td><strong>Distributive<\/strong><\/td><td>Often hyperdynamic (early)<\/td><td>Can show late-stage sepsis-induced cardiomyopathy<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p class=\"wp-block-paragraph\">M-Mode echocardiogram showing EPSS or E point septal separation of the mitral valve echo. E point is the maximum opening motion of the anterior mitral leaflet. Normally it will reach almost up to the septum, with only minimal separation. Increased EPSS means mitral valve opening excursions are low due to low cardiac output state. This M-Mode also shows poor left ventricular contraction as evidenced by little movement of IVS towards LVPW. B hump in the AML motion indicates high left ventricular end diastolic pressure in heart failure. Pericardial effusion marked as PE appears as echo free space around the heart.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">2. The Tank: Volume and Effusions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The &#8220;Tank&#8221; assessment looks at the Inferior Vena Cava (IVC) for volume status and &#8220;leaks&#8221; (free fluid).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>The IVC:<\/strong>\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Flat IVC (&lt;1.5 cm) + High Collapsibility:<\/strong> Suggests Hypovolemic or Distributive shock.<\/li>\n\n\n\n<li class=\"\"><strong>Distended IVC (>2.1 cm) + No Collapse:<\/strong> Suggests Cardiogenic or Obstructive (tamponade\/pulmonary embolism) shock.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"\"><strong>The &#8220;Leaks&#8221; (E-FAST &#8211; Extended Focused Assessment with Sonography in Trauma):<\/strong>\n<ul class=\"wp-block-list\">\n<li class=\"\">Check Morrison\u2019s Pouch, the Splenorenal space, and the Pelvis for free fluid (hemorrhage).<\/li>\n\n\n\n<li class=\"\"><strong>Lungs:<\/strong> Check for <strong>B-lines<\/strong> (pulmonary edema, typical of Cardiogenic shock) vs. <strong>Lung Sliding<\/strong> (absent in pneumothorax\u2014a cause of Obstructive shock). <\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">3. The Pipes: Great Vessels<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">This step identifies catastrophic vascular failures.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Aorta:<\/strong> Scan for Abdominal Aortic Aneurysm (AAA) or intimal flaps (Aortic Dissection).<\/li>\n\n\n\n<li class=\"\"><strong>Veins:<\/strong> Perform a two-point compression scan (femoral and popliteal veins) to look for DVT, which increases the probability of Pulmonary Embolism (Obstructive shock).
