May be heard in normal  young children.<\/td><\/tr><tr><td><strong>S4 (Atrial Gallop)<\/strong><\/td><td>Late diastole (atrial kick)<\/td><td>Low pitch (Bell)<\/td><td><strong>&#8220;Sound of Stress&#8221;<\/strong>: Associated with stiff\/hypertrophic ventricles (AS, HTN, HOCM). <em>Never heard in Atrial Fibrillation.<\/em><\/td><\/tr><tr><td><strong>Opening Snap (OS)<\/strong><\/td><td>Early diastole (after S2)<\/td><td>High pitch (Diaphragm)<\/td><td><strong>Mitral Stenosis<\/strong>: A shorter S2-OS interval indicates more severe stenosis (higher LA pressure).<\/td><\/tr><tr><td><strong>Pericardial Knock<\/strong><\/td><td>Early diastole<\/td><td>High pitch<\/td><td><strong>Constrictive Pericarditis<\/strong>: Occurs earlier and is sharper than an S3; represents the sudden cessation of ventricular filling.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">2. Advanced Murmur Morphology<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The &#8220;shape&#8221; of a murmur on a phonocardiogram reflects the pressure gradient across the valve.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Crescendo-Decrescendo (Ejection):<\/strong> Typical of <strong>Aortic Stenosis (AS)<\/strong>. The later the peak of the murmur, the more severe the stenosis, as it takes longer for the ventricle to generate peak pressure against the narrowed orifice.<\/li>\n\n\n\n<li class=\"\"><strong>Holosystolic (Plateau):<\/strong> Characteristic of <strong>Mitral Regurgitation (MR)<\/strong> or VSD. The pressure gradient between the LV and LA remains high throughout the entire systolic phase.<\/li>\n\n\n\n<li class=\"\"><strong>Decrescendo (Regurgitant):<\/strong> Typical of <strong>Aortic Regurgitation (AR)<\/strong>. It begins immediately after S2 and fades as the pressure in the aorta drops and the LV fills.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">3. Dynamic Auscultation: The &#8220;Stress Test&#8221; of the Stethoscope<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Physical maneuvers are essential when two murmurs sound similar (e.g., AS vs. HOCM or MR vs. TR).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Preload &amp; Afterload Maneuvers<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Valsalva (Strain Phase):<\/strong> Decreases preload.\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Decreases<\/strong> most murmurs (AS, MR).<\/li>\n\n\n\n<li class=\"\"><strong>Increases<\/strong> the murmur of <strong>HOCM<\/strong> (smaller LV volume allows for more obstruction) and moves the <strong>MVP<\/strong> click earlier.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"\"><strong>Squatting:<\/strong> Increases both preload and afterload.\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Increases<\/strong> AR, MR, and AS.<\/li>\n\n\n\n<li class=\"\"><strong>Decreases<\/strong> HOCM and moves the MVP click later.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"\"><strong>Handgrip:<\/strong> Increases afterload (peripheral resistance).\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Increases<\/strong> left-sided regurgitant murmurs (MR, AR).<\/li>\n\n\n\n<li class=\"\"><strong>Decreases<\/strong> the murmur of AS and HOCM.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"\"><strong>Respiration (Carvallo\u2019s Sign):<\/strong> * <strong>Inspiration<\/strong> increases right-sided murmurs (TR, PS) due to increased venous return.\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Expiration<\/strong> accentuates left-sided murmurs (MR, AR, AS).<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">4. Complex Clinical Phenomena<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Gallavardin Phenomenon:<\/strong> In elderly patients with calcific AS, the harsh systolic murmur may be heard as a high-pitched, musical sound at the apex, mimicking MR. Differentiate using <strong>handgrip<\/strong> (MR increases, AS does not).<\/li>\n\n\n\n<li class=\"\"><strong>Austin Flint Murmur:<\/strong> A mid-diastolic rumble at the apex heard in severe <strong>Aortic Regurgitation<\/strong>. It is caused by the AR jet displacing the mitral valve leaflet, creating a &#8220;functional&#8221; mitral stenosis.<\/li>\n\n\n\n<li class=\"\"><strong>Summation Gallop:<\/strong> When a patient has both S3 and S4, usually during tachycardia, the sounds fuse into one loud diastolic sound.<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Clinical Tip:<\/strong> Always palpate the <strong>Carotid Pulse<\/strong> while auscultating.
