Because the RV is highly sensitive to afterload and preload and has a complex, crescentic geometry, a multi-parametric approach is necessary.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here is a streamlined protocol to assess RV function efficiently.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">1. Visual Assessment &amp; Dimensions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Start with the <strong>Apical 4-Chamber (A4C)<\/strong> view to gauge the &#8220;big picture.&#8221;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>RV\/LV Ratio:<\/strong> In a normal heart, the RV should be about 2\/3 the size of the LV.\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Mild Dilation:<\/strong> RV is larger than 2\/3 but smaller than the LV.<\/li>\n\n\n\n<li class=\"\"><strong>Severe Dilation:<\/strong> RV is larger than the LV or forms the apex of the heart.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li class=\"\"><strong>McConnell\u2019s Sign:<\/strong> Look for akinesia of the RV free wall with sparing of the apex\u2014a classic (though not pathognomonic) sign of acute Pulmonary Embolism.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">2. Longitudinal Function (TAPSE)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Longitudinal contraction accounts for the majority of RV stroke volume.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Measurement:<\/strong> Use <strong>M-Mode<\/strong> in the A4C view. Place the cursor through the lateral tricuspid annulus.<\/li>\n\n\n\n<li class=\"\"><strong>Normal Threshold:<\/strong> >17 mm.<\/li>\n\n\n\n<li class=\"\"><strong>Clinical Pearl:<\/strong> TAPSE is highly angle-dependent. Ensure your cursor is parallel to the RV free wall motion.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">3. Radial Function (FAC)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Fractional Area Change (FAC) provides a global estimate of RV systolic function by comparing end-diastolic and end-systolic areas.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">FAC = {[RV Area (Diastole) &#8211; RV Area (Systole)] \/ RV Area (Diastole)} x 100<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Normal Threshold:<\/strong> >35%.<\/li>\n\n\n\n<li class=\"\"><strong>Efficiency Tip:<\/strong> If the RV endocardium is difficult to trace, focus on visual &#8220;eyeballing&#8221; of inward wall motion in the <strong>Parasternal Short Axis (PSAX)<\/strong>.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">4. Pressure Overload (The &#8220;D-Sign&#8221;)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Switch to the <strong>Parasternal Short Axis (PSAX)<\/strong> view at the level of the papillary muscles.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Septal Flattening:<\/strong> In states of RV pressure or volume overload, the interventricular septum flattens, turning the LV from an &#8220;O&#8221; shape into a &#8220;D&#8221; shape.\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Diastolic Flattening:<\/strong> Suggests volume overload.<\/li>\n\n\n\n<li class=\"\"><strong>Systolic Flattening:<\/strong> Suggests pressure overload (e.g., Pulmonary Hypertension).<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">5.
