In individuals with these conditions, the risk is significantly higher than in the general population due to specific physiological changes.<sup><\/sup><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why the Risk Increases<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In a healthy heart, blood traveling from the body passes through the lungs, where the pulmonary capillary bed acts as a biological filter, trapping bacteria before the blood returns to the brain. In CCHD, two main factors bypass this safety mechanism:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Right-to-Left Shunt:<\/strong> Deoxygenated blood bypasses the lungs and moves directly into the systemic circulation. This allows bacteria (which might otherwise be filtered out) to reach the brain directly via the carotid arteries.<\/li>\n\n\n\n<li class=\"\"><strong>Hypoxia and Polycythemia:<\/strong> Chronic low oxygen levels cause the body to produce more red blood cells (polycythemia), making the blood more viscous. This can lead to small areas of &#8220;micro-infarction&#8221; or tissue death in the brain, creating a perfect, low-oxygen environment for anaerobic bacteria to grow.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Clinical Presentation<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The &#8220;classic triad&#8221; of brain abscess is often incomplete, but clinicians look for:<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li class=\"\"><strong>Headache:<\/strong> Often the most common and earliest symptom.<\/li>\n\n\n\n<li class=\"\"><strong>Fever:<\/strong> Present in only about half of cases.<\/li>\n\n\n\n<li class=\"\"><strong>Focal Neurological Deficits:<\/strong> Such as weakness on one side, vision changes, or seizures.<\/li>\n<\/ol>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Note:<\/strong> Because children with CCHD may already have baseline fatigue or cyanosis, a new-onset headache or a change in mental status should always be treated as a red flag.<\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Diagnostic and Management Approach<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If a brain abscess is suspected, the following steps are typically prioritized:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">1. Imaging<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Contrast-enhanced CT or MRI<\/strong>. MRI is generally preferred for its ability to detect early &#8220;cerebritis&#8221; (the stage before a full abscess forms) and to differentiate an abscess from a tumor. Please note that contrast administration for contrast enhanced CT has additional risks in a cyanotic infant.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">2. Microbiology<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Common pathogens involved are often anaerobic or microaerophilic, such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><em>Streptococcus viridans<\/em><\/li>\n\n\n\n<li class=\"\"><em>Staphylococcus aureus<\/em><\/li>\n\n\n\n<li class=\"\"><em>Haemophilus<\/em> species<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">3.
