Conversely, high central venous pressure (venous congestion) increases renal interstitial pressure, reducing the glomerular filtration rate (GFR).<\/li>\n\n\n\n<li class=\"\"><strong>RAAS Activation:<\/strong> Both heart and kidney failure trigger the <strong>Renin-Angiotensin-Aldosterone System<\/strong>, causing sodium\/water retention and systemic vasoconstriction, which increases the workload on the heart.<\/li>\n\n\n\n<li class=\"\"><strong>Sympathetic Nervous System (SNS):<\/strong> Chronic overactivity leads to further renal vasoconstriction and cardiac remodeling.<\/li>\n\n\n\n<li class=\"\"><strong>Inflammation &amp; Oxidative Stress:<\/strong> Shared cytokines (like TNF alpha and IL-6) promote fibrosis in both tissues.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">Clinical Challenges<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Diuretic Resistance:<\/strong> As renal function declines, the kidneys become less responsive to loop diuretics, making fluid management in heart failure exceptionally difficult.<\/li>\n\n\n\n<li class=\"\"><strong>Creatinine &#8220;Bumps&#8221;:<\/strong> During treatment for acute heart failure, serum creatinine often rises as fluid is removed. Deciding whether this is &#8220;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31648696\/\" type=\"link\" id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31648696\/\">permissive&#8221; AK<\/a>I (treatment strategy to benefit long-term cardiovascular and kidney risk) or true renal damage is a common clinical dilemma.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Management Strategies<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Decongestion:<\/strong> Loop diuretics are first-line, often supplemented by thiazides or metolazone to overcome resistance.<\/li>\n\n\n\n<li class=\"\"><strong>GDMT:<\/strong> Use of ACE inhibitors, ARBs, or ARNIs (Sacubitril\/Valsartan), though these require careful monitoring of K+ and GFR. Combination of ACE inhibitors with ARB is discouraged.
