Unlike chronic mitral regurgitation, which develops over years and allows the heart time to adapt, in acute MR, the <strong>heart has no time to compensate<\/strong>. This sudden volume and pressure overload on the previously normal-sized, non-compliant left atrium rapidly leads to acute pulmonary edema and often cardiogenic shock due to a severe drop in forward cardiac output.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">1. Hemodynamics<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In acute MR, the primary issue is a <strong>sudden volume overload<\/strong> in a non-compliant system.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Left Atrial Pressure:<\/strong> The LA is small and non-compliant. The sudden regurgitant volume causes a massive spike in LA pressure, characterized by a <strong>prominent &#8216;v&#8217; wave<\/strong> on pulmonary capillary wedge pressure (PCWP) tracings.<\/li>\n\n\n\n<li class=\"\"><strong>Pulmonary Congestion:<\/strong> This high pressure is transmitted backward instantly into the pulmonary vasculature, leading to acute pulmonary edema.<\/li>\n\n\n\n<li class=\"\"><strong>Forward Cardiac Output:<\/strong> A large portion of the LV stroke volume is ejected into the low-pressure LA rather than the aorta. This leads to a drop in systemic cardiac output and, in severe cases, cardiogenic shock.<\/li>\n\n\n\n<li class=\"\"><strong>LV Function:<\/strong> Initially, the LV ejection fraction (LVEF) may appear &#8220;hyperdynamic&#8221; (high) because the ventricle is emptying into a low-pressure sink (the LA), but this masks the actual decrease in forward flow.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">2. Common Causes<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Acute MR usually results from a catastrophic structural failure of the mitral valve apparatus:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Component<\/strong><\/td><td><strong>Common Etiologies<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>Chordae Tendineae<\/strong><\/td><td>Myxomatous degeneration (MVP), Infective Endocarditis, or blunt chest trauma.<\/td><\/tr><tr><td><strong>Papillary Muscle<\/strong><\/td><td><strong>Acute Myocardial Infarction<\/strong> (typically 2\u20137 days post-MI). The posteromedial papillary muscle is most vulnerable as it usually has a single blood supply (RCA).<\/td><\/tr><tr><td><strong>Valve Leaflets<\/strong><\/td><td>Infective Endocarditis (perforation or large vegetations preventing closure).<\/td><\/tr><tr><td><strong>Prosthetic Valve<\/strong><\/td><td>Dehiscence, thrombus, or structural failure of a bioprosthetic valve.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">3. Management<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The goal of management is to <strong>reduce afterload<\/strong> (to encourage forward flow) and stabilize the patient for definitive repair.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Medical Stabilization<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Afterload Reduction:<\/strong> * <strong>Vasodilators:<\/strong> IV Sodium Nitroprusside is the gold standard if the patient is normotensive. It lowers systemic vascular resistance, making it &#8220;easier&#8221; for the LV to pump blood into the aorta than the LA.<\/li>\n\n\n\n<li class=\"\"><strong>Diuretics:<\/strong> IV Furosemide to reduce pulmonary venous congestion.<\/li>\n\n\n\n<li class=\"\"><strong>Inotropes:<\/strong> If the patient is hypotensive (cardiogenic shock), dobutamine may be used, though it must be balanced carefully as it can increase myocardial oxygen demand.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Mechanical Support<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Intra-aortic Balloon Pump (IABP):<\/strong> Highly effective for acute MR. It decreases afterload during systole (facilitating forward flow) and increases coronary perfusion during diastole.
