The disease passes through an initial phase of acute myocarditis, later leading to necrotic and thrombotic phases and finally the fibrotic phase. Clinical Presentation<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The symptoms usually reflect <strong>restrictive physiology<\/strong>, where the heart becomes stiff and cannot fill properly during diastole.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Right-Sided Failure:<\/strong> Jugular venous distension (JVD), hepatomegaly, ascites, and peripheral edema.<\/li>\n\n\n\n<li class=\"\"><strong>Left-Sided Failure:<\/strong> Dyspnea on exertion, orthopnea, and pulmonary congestion.<\/li>\n\n\n\n<li class=\"\"><strong>Arrhythmias:<\/strong> Atrial fibrillation is common due to atrial enlargement.<\/li>\n\n\n\n<li class=\"\"><strong>Thromboembolism:<\/strong> Risk of systemic or pulmonary emboli from mural thrombi.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>3. Diagnostic Framework<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Echocardiography (First-line)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The hallmark is the <strong>obliteration of the ventricular apex<\/strong> by fibrous tissue or thrombus. You will often see:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\">Marked atrial enlargement (&#8220;giant atria&#8221;).<\/li>\n\n\n\n<li class=\"\">Normal or slightly reduced systolic function with severe diastolic dysfunction.<\/li>\n\n\n\n<li class=\"\">Tethering of the atrioventricular valves (Mitral or Tricuspid regurgitation).<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Cardiac MRI (Gold Standard)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">MRI is superior for tissue characterization.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Late Gadolinium Enhancement (LGE):<\/strong> Typically shows subendocardial enhancement in a &#8220;V&#8221; shape at the apex. A <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3106963\/\" type=\"link\" id=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3106963\/\">double V<\/a> shape has also been described.<\/li>\n\n\n\n<li class=\"\"><strong>Thrombus Detection:<\/strong> Better sensitivity than echo for identifying apical clots.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Endomyocardial Biopsy<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">While definitive for seeing eosinophilic infiltration or dense collagen, it is often avoided due to the risk of dislodging thrombi or &#8220;sampling error&#8221; if the biopsy misses the fibrotic patch.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>4. Management Strategies<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Treatment focuses on symptom relief and preventing complications.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Medical:<\/strong> Diuretics for congestion and anticoagulation (warfarin) for thrombus prevention.
