While heart failure medications improve cardiac function and symptoms, they cannot always stop the chaotic electrical signals that lead to cardiac arrest. The use of an ICD is generally categorized into two &#8211; primary and secondary prevention strategies.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">1. Primary Prevention<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This applies to patients who have never had a life-threatening arrhythmia but are at high risk because of their underlying cardiac condition.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Ejection Fraction (EF) &lt; 35%:<\/strong> This is the most common threshold. The primary &#8220;role&#8221; of the ICD is now being deferred or even eliminated in many patients due to a phenomenon called <strong>Reverse Remodeling<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Reverse Remodeling and &#8220;EF Recovery&#8221;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The most significant impact of ARNI and SGLT2i is their ability to improve the Left Ventricular Ejection Fraction (LVEF) beyond the 35% threshold, rendering an ICD unnecessary.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.121.054034\" type=\"link\" id=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.121.054034\">PROVE-HF (ARNI)<\/a>:<\/strong> This trial demonstrated that sacubitril\/valsartan leads to significant rapid reverse remodeling. Among patients with HFrEF who met primary prevention ICD eligibility criteria at baseline, 62% improved their EF to >35% by 12 months after initiation of sacubitril\/valsartan therapy.<\/li>\n\n\n\n<li class=\"\"><strong>SGLT2i Impact:<\/strong> Recent evidence shows that adding an SGLT2i to an ARNI has a <strong>synergistic effect<\/strong> on ventricular volumes and mass reduction, further increasing the rate of &#8220;HFimpEF&#8221; (Heart Failure with Improved EF). SGLT2i therapy is associated with a <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/jce.15894\" type=\"link\" id=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/jce.15894\">reduced risk of sudden cardiac death<\/a> in patients with heart failure receiving contemporary medical therapy.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Change in ICD Implantation Rates<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Recent observational data indicates a downward trend in primary prevention ICD implantations.
