This is the most common variety.<\/li>\n\n\n\n<li class=\"\"><strong>Hypoplastic Mitral Stenosis:<\/strong> A miniature version of a normal valve, typically seen in Hypoplastic Left Heart Syndrome (HLHS). Second most common variety. Noted in infancy and has never been reported as a de novo diagnosis in adults.<\/li>\n\n\n\n<li class=\"\"><strong>Supramitral Ring:<\/strong> A fibrous diaphragm just above the mitral annulus (a classic part of Shone complex). Some consider this to be an acquired lesion resulting from turbulent flow across the mitral valve.<\/li>\n\n\n\n<li class=\"\"><strong>Parachute Mitral Valve:<\/strong> All chordae tendineae insert into a single, dominant papillary muscle. Though this is the least common form of congenital mitral stenosis, it is discovered the most frequently as a de novo diagnosis in the adult population.<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">## Shone Complex<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">CMS is frequently a component of <strong>Shone Complex<\/strong>, a series of obstructive lesions on the left side of the heart. <\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li class=\"\">Supramitral ring.<\/li>\n\n\n\n<li class=\"\">Parachute mitral valve.<\/li>\n\n\n\n<li class=\"\">Subaortic stenosis.<\/li>\n\n\n\n<li class=\"\">Coarctation of the aorta.<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">## Clinical Presentation<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The severity of symptoms depends on the degree of obstruction and the presence of associated shunts (like an ASD, which can &#8220;decompress&#8221; the left atrium).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Symptoms:<\/strong> Tachypnea, poor feeding, failure to thrive, and recurrent pulmonary infections (due to pulmonary venous congestion).<\/li>\n\n\n\n<li class=\"\"><strong>Physical Exam:<\/strong> * Loud S<sub>1<\/sub> and an apical mid-diastolic rumble.\n<ul class=\"wp-block-list\">\n<li class=\"\">Unlike rheumatic MS, an <strong>opening snap is usually absent<\/strong> because the leaflets are thick and non-pliable rather than fused at the tips.<\/li>\n\n\n\n<li class=\"\">Signs of right ventricular hypertrophy (RV heave) if pulmonary hypertension has developed.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">## Diagnostic Evaluation<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">### Echocardiography (Gold Standard)<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Assessment focuses on the <strong>Simpson\u2019s Biplane<\/strong> method for volumes and specific Doppler hemodynamics:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Mean Pressure Gradient:<\/strong> Elevated gradients (often > 5-10 mmHg) indicate significant stenosis.<\/li>\n\n\n\n<li class=\"\"><strong>Pressure Half-Time (T<sub>1\/2<\/sub>):<\/strong> Used to calculate the Mitral Valve Area (MVA), though less reliable in infants with high heart rates.<\/li>\n\n\n\n<li class=\"\"><strong>Morphology:<\/strong> Identification of &#8220;parachute&#8221; deformity or supramitral rings.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">### Cardiac Catheterization<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Reserved for cases where non-invasive imaging is inconclusive or to precisely measure <strong>Pulmonary Capillary Wedge Pressure (PCWP)<\/strong> and pulmonary vascular resistance prior to surgery.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">## Management Strategies<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">### Medical Management<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Primarily supportive to manage pulmonary congestion:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Diuretics:<\/strong> To reduce preload and pulmonary edema.<\/li>\n\n\n\n<li class=\"\"><strong>Afterload reduction:<\/strong> Generally avoided as it may compromise systemic perfusion in the setting of fixed inflow obstruction.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">### Surgical Intervention<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Surgery is challenging due to the small size of the valve and the risk of inducing mitral regurgitation.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Mitral Valvuloplasty:<\/strong> Commissurotomy or resection of accessory chordae\/supramitral rings.<\/li>\n\n\n\n<li class=\"\"><strong>Valve Replacement:<\/strong> Used only as a last resort in infants.
