The current landscape of IVUS can be summarized through its evidence base, technical parameters, and clinical optimization criteria.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Current Clinical Evidence &amp; Guidelines<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The latest consensus, reinforces IVUS as a <strong>Class 1A recommendation<\/strong> for guiding PCI in left main (LM) and complex lesions.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>DKCRUSH VIII:<\/strong> Demonstrated that in true complex bifurcation lesions, IVUS-guided PCI significantly reduced target-vessel failure (TVF) compared to angiography alone.<\/li>\n\n\n\n<li class=\"\"><strong>IVUS-CHIP &amp; OPTIMAL:<\/strong> These trials presented a more nuanced view, suggesting that in the hands of high-volume, expert operators, angiography can achieve comparable results to IVUS in certain high-risk\/LM cases. An arc > 180\u00b0 often necessitates advanced lesion preparation (Lithotripsy or Rotablation).<\/li>\n\n\n\n<li class=\"\"><strong>Plaque Characterization:<\/strong> Distinguishing between <strong>soft (lipid-rich)<\/strong>, <strong>fibrous<\/strong>, and <strong>calcified<\/strong> plaque guides the choice of inflation pressure and balloon type.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">DKCRUSH VIII<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>DKCRUSH VIII<\/strong> trial was a landmark study presented at the <strong>ACC.26 Scientific Sessions<\/strong> in New Orleans and simultaneously published in the <a href=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2026.01.081\" type=\"link\" id=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2026.01.081\"><em>Journal of the American College of Cardiology<\/em> (JACC) in March 2026<\/a>. The study randomized 555 patients with complex coronary bifurcation lesions (primarily defined by the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25326748\/\" type=\"link\" id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25326748\/\">DEFINITION<\/a> criteria) to either <strong>IVUS-guided<\/strong> or <strong>angiography-guided<\/strong> PCI using the <strong>DK crush<\/strong> technique.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Key Findings<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Primary Endpoint:<\/strong> At 1 year, the rate of target vessel failure (TVF) was significantly lower in the IVUS-guided group compared to the angiography group (<strong>6.1% vs. 14.7%<\/strong>; HR 0.40; p = 0.002).<\/li>\n\n\n\n<li class=\"\"><strong>Clinical Impact:<\/strong> The benefit was largely driven by reductions in target vessel myocardial infarction (TVMI) and clinically driven target vessel revascularization (TVR).<\/li>\n\n\n\n<li class=\"\"><strong>Mechanism:<\/strong> The investigators concluded that the superior outcomes were achieved by reaching specific <strong>IVUS-defined optimization targets<\/strong>, rather than the mere use of the imaging tool itself.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\"><a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2601521\" type=\"link\" id=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2601521\">IVUS-CHIP<\/a><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Focus:<\/strong> Complex high-risk indicated procedures (CHIP) in 2,020 patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Finding:<\/strong> Routine IVUS guidance was <strong>not superior<\/strong> to angiography alone in reducing target-vessel failure (TVF). TVF occurred in 14% of the IVUS group vs. 11% of the angiography group (p = 0.08).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><a href=\"https:\/\/www.nejm.org\/doi\/abs\/10.1056\/NEJMoa2600440\" type=\"link\" id=\"https:\/\/www.nejm.org\/doi\/abs\/10.1056\/NEJMoa2600440\">OPTIMAL<\/a><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Focus:<\/strong> Unprotected Left Main (LM) coronary artery disease in 806 patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Finding:<\/strong> IVUS guidance yielded <strong>similar results<\/strong> to angiography for the primary endpoint (death, MI, stroke, or revascularization) at a median follow-up of 2.9 years (34% vs. 31%; p = 0.40).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The IVUS-CHIP and OPTIMAL trials were both presented as Late-Breaking Clinical Trials at the ACC.26 Scientific Sessions in March 2026 and simultaneously published in the New England Journal of Medicine (NEJM). Notably, both trials were neutral, showing that in the hands of expert operators at high-volume centers, IVUS-guided PCI did not significantly reduce the primary composite endpoints compared to angiography-guided PCI.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">RENOVATE-COMPLEX-PCI<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2216607\" type=\"link\" id=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2216607\">RENOVATE-COMPLEX-PCI<\/a><\/strong> (Randomized Controlled Trial of Intravascular Imaging-Guided vs. Angiography-Guided Percutaneous Coronary Intervention in Complex Coronary Artery Lesions) trial has provided some of the strongest evidence for the long-term benefits of intravascular imaging in complex cases. <strong><a href=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2026.01.035\" type=\"link\" id=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2026.01.035\">5-Year Long-Term Outcomes<\/a><\/strong>: Presented at the <strong>ACC.26 Scientific Sessions<\/strong>, this extended follow-up confirmed that the benefits of imaging guidance are sustained over the long term without a &#8220;late catch-up&#8221; phenomenon.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">ULTIMATE<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>ULTIMATE<\/strong> trial (Intravascular Ultrasound Versus Angiography-Guided Drug-Eluting Stent Implantation in &#8220;All-Comers&#8221; Coronary Lesions) is a landmark multicenter, randomized controlled trial that established the clinical benefit of IVUS in a general patient population, rather than just specific complex subgroups. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Original 12-Month Results:<\/strong> <strong><a href=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2018.09.013\" target=\"_blank\" rel=\"noreferrer noopener\">Intravascular Ultrasound Versus Angiography-Guided Drug-Eluting Stent Implantation: The ULTIMATE Trial<\/a><\/strong> <em>DOI: 10.1016\/j.jacc.2018.09.013<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Long-Term 3-Year Outcomes:<\/strong> <strong><a href=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jcin.2020.10.001\" target=\"_blank\" rel=\"noreferrer noopener\">3-Year Outcomes of the ULTIMATE Trial<\/a><\/strong> <em>DOI: 10.1016\/j.jcin.2020.10.001<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Coronary Intravascular Ultrasound (IVUS) remains a cornerstone of precision PCI, particularly as recent clinical data continues to refine its role in complex substrates.
