Because this initial deflection represents purely pre-excited myocardium before normal His-Purkinje conduction catches up, the vector of the delta wave points directly <em>away<\/em> from the pathway&#8217;s ventricular insertion site.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here is the step-wise approach to pinpointing the pathway along the atrioventricular annuli.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">1. Left vs. Right (The V1 Discriminator)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Lead V1 sits to the right and anteriorly. It is the primary discriminator for which side of the heart holds the pathway.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>V1 Morphology<\/strong><\/td><td><strong>Pathway Location<\/strong><\/td><td><strong>Vector Direction<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>Positive Delta (R &gt; S)<\/strong><\/td><td>Left-sided (Classic &#8220;Type A&#8221;)<\/td><td>Travels anteriorly and rightward toward V1<\/td><\/tr><tr><td><strong>Negative Delta (QS or rS)<\/strong><\/td><td>Right-sided (Classic &#8220;Type B&#8221;)<\/td><td>Travels posteriorly and leftward away from V1<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">2. Anterior vs. Posterior (The Inferior Leads)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Leads II, III, and aVF act as the floor of the heart. They determine the pathway&#8217;s position along the superior-inferior axis.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Inferior Leads (II, III, aVF)<\/strong><\/td><td><strong>Pathway Location<\/strong><\/td><td><strong>Vector Direction<\/strong><\/td><\/tr><\/thead><tbody><tr><td><strong>Negative Delta<\/strong><\/td><td>Posterior \/ Posteroseptal<\/td><td>Travels superiorly, away from the inferior leads<\/td><\/tr><tr><td><strong>Positive Delta<\/strong><\/td><td>Anterior \/ Anteroseptal<\/td><td>Travels inferiorly, toward the diaphragmatic surface<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">3. Septal vs. Lateral Free Wall<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Differentiating a septal insertion from a free-wall insertion requires looking at the frontal plane leads (I and aVL) and the precordial transition zone.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Left Lateral Free Wall:<\/strong> Look for a negative or isoelectric delta wave in leads I and aVL. The impulse is traveling from the far left lateral edge toward the right.<\/li>\n\n\n\n<li class=\"\"><strong>Right Lateral Free Wall:<\/strong> Typically shows a strongly positive delta wave in lead I. The precordial transition (R > S) is delayed, often not occurring until V3 or V4.<\/li>\n\n\n\n<li class=\"\"><strong>Septal Pathways:<\/strong> These often present with a very early precordial transition (V1 or V2). Posteroseptal pathways specifically will often mimic an inferior myocardial infarction, presenting with deep Q waves (which are actually negative delta waves) in leads II, III, and aVF.<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Clinical Caveat:<\/strong> Accurate localization requires maximal pre-excitation.
