Intervention is typically deferred until the risk of rupture\u2014which is remarkably low for small AAAs\u2014outweighs the procedural risks of endovascular aneurysm repair (EVAR) or open surgery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Elective repair is generally indicated when the AAA reaches:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\">5.5 cm in men<\/li>\n\n\n\n<li class=\"\">5.0 cm in women<\/li>\n\n\n\n<li class=\"\">A rapid expansion rate of 0.5 cm or more within 6 months<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Imaging Surveillance Intervals<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Surveillance relies primarily on ultrasound to track the maximum anteroposterior diameter. Computed tomography angiography (CTA) is generally reserved for treatment planning once the diameter threshold has been met, or if rupture is suspected.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The <a href=\"https:\/\/www.ejves.com\/article\/S1078-5884(23)00889-4\/fulltext\" type=\"link\" id=\"https:\/\/www.ejves.com\/article\/S1078-5884(23)00889-4\/fulltext\">2024 European Society for Vascular Surgery (ESVS) guidelines<\/a> recommend the following gender-specific screening intervals:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><td><strong>Aortic Diameter<\/strong><\/td><td><strong>Men<\/strong><\/td><td><strong>Women<\/strong><\/td><\/tr><\/thead><tbody><tr><td>25 to 29 mm<\/td><td>Every 5 years<\/td><td>Every 5 years<\/td><\/tr><tr><td>30 to 39 mm<\/td><td>Every 3 years<\/td><td>Every 3 years<\/td><\/tr><tr><td>40 to 44 mm<\/td><td>Annually<\/td><td>Annually<\/td><\/tr><tr><td>45 to 49 mm<\/td><td>Annually<\/td><td>Every 6 months<\/td><\/tr><tr><td>50 to 54 mm<\/td><td>Every 6 months (\u2265 55 mm elective repair)<\/td><td>Repair indicated (\u2265 50 mm elective repair)<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Note: ACC\/AHA 2022 guidelines offer closely aligned intervals, typically recommending 3-year intervals for 3.0 to 3.9 cm, annual imaging for 4.0 to 4.9 cm, and 6-month imaging for 5.0 to 5.4 cm.<\/em><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Medical and Lifestyle Management<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Pharmacotherapy and lifestyle modifications focus on reducing overall cardiovascular morbidity, though no specific medication has been definitively proven to halt AAA expansion.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Smoking Cessation:<\/strong> This is the most critical modifiable risk factor. Continued smoking is directly correlated with accelerated expansion rates and a significantly higher risk of rupture.<\/li>\n\n\n\n<li class=\"\"><strong>Cardiovascular Risk Reduction:<\/strong> Standard secondary prevention protocols apply, requiring strict blood pressure control, high-intensity statin therapy, and antiplatelet therapy.<\/li>\n\n\n\n<li class=\"\"><strong>Physical Activity:<\/strong> Routine exercise do not need to be restricted in patients with small, asymptomatic AAAs.<\/li>\n\n\n\n<li class=\"\"><strong>Fluoroquinolones:<\/strong> While the FDA previously issued warnings regarding fluoroquinolone use in patients with aortic disease, the <a href=\"https:\/\/www.ejves.com\/article\/S1078-5884(23)00889-4\/fulltext\" type=\"link\" id=\"https:\/\/www.ejves.com\/article\/S1078-5884(23)00889-4\/fulltext\">2024 ESVS guidelines clarify that a small AAA is not a strict contraindication<\/a> if these antibiotics are otherwise clinically necessary.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Intervention Thresholds Non-surgical (Conservative) management is the standard of care for asymptomatic abdominal aortic aneurysms (AAAs) that fall below the threshold for elective repair.
