Early implantation in this window has failed to demonstrate a primary prevention survival benefit (wearable cardioverter-defibrillators are typically utilized as a bridge).<\/li>\n\n\n\n<li class=\"\"><strong>Recent Revascularization:<\/strong> Within 90 days of CABG or PCI, to allow adequate time for myocardial recovery and reassessment of the LVEF.<\/li>\n\n\n\n<li class=\"\"><strong>Transient Triggers:<\/strong> VT or VF strictly due to completely reversible disorders\u2014such as acute ischemia, severe electrolyte derangements, or proarrhythmic drug toxicity\u2014where correction is feasible and likely to prevent recurrence.<\/li>\n\n\n\n<li class=\"\"><strong>Ablatable Arrhythmias:<\/strong> VT or VF driven by mechanisms highly amenable to catheter or surgical ablation, such as Wolff-Parkinson-White syndrome, RVOT VT, idiopathic LV fascicular VT, or typical bundle branch reentrant VT.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">End-Stage or Terminal Conditions<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Refractory Heart Failure:<\/strong> NYHA Class IV heart failure symptoms that are refractory to guideline-directed medical therapy, <em>unless<\/em> the patient is an active candidate for CRT-D, cardiac transplantation, or mechanical circulatory support (LVAD).<\/li>\n\n\n\n<li class=\"\"><strong>Limited Life Expectancy:<\/strong> Any systemic illness (e.g., metastatic malignancy, end-stage organ failure) or irreversible neurological damage that limits life expectancy with an acceptable functional status to less than 1 year.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Inappropriate Arrhythmia Presentations<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"\"><strong>Incessant Arrhythmias:<\/strong> Incessant VT or VF.
