My answer is number 1. Only about 10% of VTs are idiopathic (no structural heart disease). And some new studies using high-resolution MRIs have suggested that the percentage may even be lower than 10%. The most common structural abnormality is scarring, usually due to a previous MI but may also be due to connective tissue disease, sarcoidosis, ARVD or even fibrosis of aging.
