Agatston score is usually measured from plain (non contrast enhanced) computed tomography (CT). Density of calcium is measured in Hounsfield units (HU). It is used to calculate a weighted value.

Weighted value for the calcified region will be 1 for 130–199 HU, 2 for 200–299 HU, 3 for 300–399 HU, and 4 for 400 HU or higher. The weighted value is then multiplied by the area in millimeters of the calcification. Total coronary artery calcium score (CAC score) is calculated as a sum of all the scores obtained in the multiple 3 mm CT slices starting from the coronary ostia to the inferior wall of the heart. A zero CAC score has an excellent prognosis. CAC above 400 has a higher risk of death even in elderly who frequently have calcification.
