Increase in venous return and consequent increase in preload on assuming the supine position is the reason for decubitus angina. Most likely these patients have severe multivessel coronary artery disease or critical narrowing of a major coronary branch.

Angina due to coronary vasospasm is known as Prinzmetal’s angina. Unlike usual angina which manifests ST segment depression during pain, Prinzmetal’s angina manifests ST elevation on ECG during pain. But unlike in ST elevation myocardial infarction, ST segment returns to baseline when vasospasm is relieved and chest pain subsides.
