Transluminal balloon tearing of discrete membranous subaortic stenosis

Discrete membranous subaortic stenosis can be treated by transluminal balloon tearing of the membrane instead of open surgical resection. Now we have a quarter century study of seventy six patients with thin discrete subaortic stenosis who underwent percutaneous transluminal balloon tearing of the membrane by José Suárez de Lezo, Miguel Romero, José Segura, Manuel Pan, Javier Suárez de Lezo, Djordje Pavlovic, Francisco Mazuelos, Mónica Delgado and Dolores Mesa [1]. The follow up period was quite long, at 16±6 years. The age group at presentation varied from two to sixty seven years. Immediate reduction in gradient achieved was 70±27 to 18±12 mm Hg. Restenosis developed in fifteen percent of them while four percent developed muscular obstructive disease and one developed a new separate membrane with obstruction. Sixteen percent underwent a repeat dilatation while five percent underwent surgical resection. More than three fourth of the patients were alive and free of redilation or surgery at follow up. Better results were noted in those with larger diameter of the aortic annulus and thinner membranes.
