GISSI trial showed that there is no benefit by giving heparin for preventing coronary reocclusion after thrombolysis with streptokinase. Heparin may be given to prevent mural thrombus formation in large infarctions. It may be considered in those with heart failure or atrial fibrillation to prevent thromboembolic complications.
