ECG shows a wide QRS rhythm at 60/min. Each QRS complex is preceded by a pacing spike. So it is a ventricular paced rhythm. Intermittent P waves are seen with totally varying PR interval, suggesting AV dissociation. So it is a single chamber ventricular pacing without any atrial sensing. Lead I and aVL shows left bundle branch block pattern and there is left axis deviation with negative QRS complexes in inferior leads II, III and aVF. This suggests pacing from right ventricular apex.
