Which cardiac chambers are seen as enlarged in this chest X-ray?
Double atrial density suggesting left atrial enlargement is seen within the right cardiac border. The right atrium is also enlarged, considering the significant shift of the right cardiac border to the right.
The distance from the midline to the right cardiac border is increased, another feature of right atrial enlargement. Right descending pulmonary artery shadow is also a little prominent, suggesting pulmonary arterial hypertension.Correspondingly, pulmonary bay is obliterated, indicating mild enlargement of main pulmonary artery as well. There is straightening of left border. Overall features would suggest mitral stenosis with pulmonary hypertension.

Left appendage is flattened out, double shadow sign – left atrial enlargement ?
I will have gone for the above comment, but rethinking of it, i dont see to much of a congestion, anyway i was thinkig of pericardial effusion. Let us know
Left atrial double shadow is clearly seen within the right border. Right atrium is also enlarged, as the right border is way beyond the right side of spine.
You are right. But the right atrium is also enlarged, considering the distance from the mid line to the right cardiac border.

pericardial effusion,as pulm trunk not enlarged n no lung congestion ,so both against the favour of left atrial enlargement.
