Clinical arrhythmia score for intra-atrial reentrant tachycardia (IART)
Came across an interesting clinical arrhythmia score for assessing severity of intra-atrial reentrant tachycardia (IART) [1]. Four parameters were considered for scoring: Documented IART, Frequency of cardioversion in the preceding three months, Severity of IART, and Antiarrhythmic Medications. Each category was scored from 0-3 so that the maximum total sum of scores would be 12.
For Documented IART, zero for none, one point for nonsustained IART only, two points for history of sustained IART and three points for incessant IART.
Frequency of cardioversion: zero for no cardioversion, one point for one cardioversion and AAIT cardioversion. Three points for two or more cardioversions. AAIT cardioversions were defined as one or more automatic or manual cardioversions performed using an implanted atrial pacemaker and not requiring any additional intervention.
In IART severity, if there were no symptoms, it was zero points. Palpitations alone would get 1 point, Syncope/Congestive Heart Failure/thrombosis would give 2 points. Occurrence of cardiac arrest gave 3 points.
Antiarrhythmic therapy: zero for none or digoxin only. One point for class II or IV antiarrhythmic therapy, two points for class I or III antiarrhythmic therapy. If there was amiodarone toxicity, that would get 3 points. Total score was calculated as a sum of the highest score in each category.
