While transthoracic echocardiography can detect vegetations as small as 2-4 mm, transesophageal echocardiography can detect smaller vegetations in the range of 1-1.5 mm. MRI and CT are not routine investigations for detection of vegetations. Absence of vegetations does not exclude infective endocarditis. If clinical suspicion is high serial echocardiography and blood cultures are needed.
