Katz-Wachtel phenomenon, which includes findings a-c. Characterized by huge voltages in the limb leads with QRS complexes in the mid-precordial leads that are essentially equiphasic. Usually due to a congenital defect such as a VSD and typically includes biventricular hypertrophy.
ECG is Katz wachtel phenomenon due to large amplitude qrs deflection in midprecordial leads in VSD suggestive of biventricular hypertrophy.
ECG also show sinus tachycardia,rt axis deviation, indirect evidence of right atrial abnormality (qrs voltage is more than thrice in V2 than V1) and evidence of lv strain in lateral leads
Rate: ~150/min
Rhythm: NSR
Axis: ~+120° ie. Right Axis Deviation
Features of Biventricular Hypertrophy: Large R waves and Deep S waves in V2 to V5 with ST depression and T wave inversion. Therefore this is an ECG of Katz Wachtel Phenomenon .. commonly seen in VSD
