On this ECG Quiz: Only a rhythm strip is given. Hence one should be concentrating on the rhythm analysis. Still any striking abnormality of the waves which can occur in lead II could also be looked into. For example, P waves are better assessed in this lead. Hence abnormalities of P waves can be made out easily. QRS width can also be measured well in this lead.

Mobitz type 2 block
Irregular sinus rhythm with a rate around 100/min – probable sinus arrhythmia. There is RP/PR reciprocity (if the RP interval shortens, the following PR interval lengthens). This tells me that there is no extraneous parasympathetic input and that the AV node appears to be functioning properly. The QRS complexes remain the same. There is a sinus pause at the end that is not a multiple of the previous RR interval (though, given the sinus irregularity, I suppose it could be a multiple of the “average” sinus rate). If it is not a multiple, then it is a sinus pause; if it IS a multiple, then it would represent a high-grade SA Mobitz II block. There is a very small “blip” following the last T wave on the tracing but I don’t think it represents a P wave. The QT interval appears a bit long grossly, but I am not sure what is should be – given the heart rate.
sinus arrythmia , sinus pause
Sinus tachycardia with sinus arrest.
