Features of left ventricular aneurysm

The important clinical finding of a left ventricular aneurysm is a dyskinetic pulsation near the apex beat. Dyskinetic pulsation is a see-saw pulsation, with one portion bulging out when the other portion goes in. This is better appreciated on a tangential view. ECG may show persistent ST segment elevation in a left ventricular aneurysm. X-ray chest will show a bulge along the left cardiac border in left ventricular aneurysm. Echocardiogram shows the dyskinetic region with a bulging out in systole as well as diastole of the aneurysmal region of the left ventricle.
