Floppy guide wire in obtuse marginal branch of left circumflex coronary artery

Floppy guide wire in obtuse marginal lesion

Left coronary angiogram in RAO caudal view showing tight stenosis of proximal segment of obtuse marginal branch of left circumflex coronary artery. A floppy guide wire has been introduced into the vessel and the thick floppy tip is seen at the distal region. The wire tip forms a small loop at the distal end. This is the preliminary step of percutaneous coronary angioplasty.

It is a right dominant system as LCX is not seen to be crossing the crux of the heart, the junction of atrioventricular and interventricular grooves posteriorly. Coronary guide catheter can be seen as well seated in the left main coronary artery.
