Damage to the valves in rheumatic fever follows the hemodynamic load. Mitral valve which faces the systolic pressure of the left ventricle in the closed position suffers the maximum damage. Next comes the aortic valve which faces the aortic diastolic pressure in the closed position. Least load is for pulmonary valve which only has the pulmonary diastolic pressure to resist. Hence pulmonary valve is least commonly involved in rheumatic fever. Organic tricuspid valve involvement comes in between. But often tricuspid regurgitation occurs in other left sided lesions because of severe pulmonary hypertension.
