PVCs and NSVT in structurally normal heart

Conventionally they do not fit into the high risk category, but they are an extremely common group. Even though the risk of SCD is high in the high risk group, the majority of SCD occur in those without high risk markers, just because of the sheer large number of this group. Some of those with recurrent PVCs or NSVT can develop tachycardiomyopathy. Those with less than 5000 VPC in 24 hours are unlikely to develop cardiomyopathy. No correlation was obtained between the type or location of the PVC and the chance of cardiomyopathy. Fusion should be excluded while checking for multiple morphologies. RF ablation may be considered in those with VPC and lower left ventricular ejection fraction.
