Previously ventilation perfusion scan was the test for pulmonary embolism. But the widespread availability of computed tomographic (CT) pulmonary angiography, it has become the first line diagnostic test as it gives good visualization of the extent and distribution of emboli. It is also useful in excluding the differential diagnoses like aortic dissection, pneumothorax and consolidation. Only disadvantage is the potential risk of contrast induced acute kidney injury.

In the absence of lung disease, ventilation perfusion scan may be useful in those with renal compromise as it does not induce acute kidney injury. It is also less expensive, though availability is more limited.
